REGISTRATION REQUIREMENTS Student does not need to be present for enrollment. Parent or legal guardian must complete enrollment documents and present to Registrar. Please read ENTIRE Registration requirements

A. Student’s birth certificate. (For verification only, no copy required)

B. Parent’s photo ID. Driver’s license, state-issued ID card, government ID card, valid passport or consulate issued photo ID

C. Verification of Residency: TWO DOCUMENTS FROM THE LIST BELOW In accord with State Compliance, you must attach two items listed below as required documentation of residency for enrollment for the 2016/2017 school year: (*Current documents within 30 days)  Current property tax statement, current mortgage statement or final close of escrow statement OR  Current rental property contract, lease, or payment receipt AND one of the following:  Utility start of service contract/confirmation of service, statement, or payment receipt*  Pay stub showing current address*  Correspondence from a government agency*  Voter registration

Both documents provided MUST BE CURRENT and have the name of the primary resident printed on the documents, the current address, and the date issued.

D. Immunizations Records: Translated into English - See attached requirement list ALL STUDENTS entering grades 7-12 are required to provide proof of the PERTUSSIS (Whooping cough) Vaccine booster (Tdap) given after the child's 7th birthday.

E. 10th-12thGrade Students: Must provide transcripts showing high school credit, in English. If enrolling from another school during the school year, submit withdrawal/checkout forms/grades from previous school with transcript.

CAREGIVER AFFIDAVIT – must be completed prior to enrollment Students who live with and are under the care of a family member other than the parents or legal guardian (Caregiver) may enroll in the school attendance area in which the caregiver lives. The caregiver must schedule an appointment with the Office of Student Support Services and com- plete a Caregiver Affidavit. The student must live full time with the caregiver to qualify for Caregiver status. The Parent/Guardian must present the student's birth certificate or passport at this appointment. Please download, follow the instructions, and complete the Caregiver's Autho- rization Form located in the "Student Support Services Supplemental Documents" link on the FUSD website. DECLARATION OF RESIDENCY – must be completed prior to enrollment Families who live in a residence other than their home residence (i.e. living with another member of the family or living with a friend) must come to Student Support Services and complete a Declaration of Residency. In order to complete this, the owner/leasee must be present at the time of the appointment. In addition, the owner/leasee must show two proofs of residency (e.g. current rental agreement/mortgage /property tax, a recent utility bill, pay stub, voter registration, or correspondence from a government agency). The parent/guardian and owner/leasee must also present photo identification (e.g. government issued identification). The Parent/Guardian must present the student's birth certificate or passport at this appointment. Please download, follow the instructions, and complete the Declaration of Residency Form located in the "Student Support Services Supplemental Documents" link on the FUSD website.

An appointment must be scheduled with Student Support Services (510- 659-2534) in order to complete a Declaration of Residency. Appointments can be made on the FUSD website. Ap- pointments cannot be made over the telephone and they do not accept walk-in appointments.

SPECIAL SERVICES (IEP) If the student was previously in Special Education classes, please go directly to FUSD Special Services Office to sign a 30 day placement request. Take a copy of the current IEP with you, if possible. You may also call them at 510-659-2569 for assistance.

Transportation Bus transportation is not provided by FUSD for high schools. Contact Alameda County Transit at actransit.org for information on routes, fares and student discounts. See the link on the AHS website for CLIPPER CARD info. The AC transit bus drops off directly in front of the school. Child Nutrition Visit the Child Nutrition link from the IHS website or FUSD website to get information on breakfast & lunch menus and pricing, pre-payment options and free/reduced lunch forms.
